CAPM Practice: Determine how to document project controls of predictive, plan-based projects

Significant unauthorized scope additions require a structured response: (1) Document each addition as a retroactive change request — don't ignore or hide them; (2) Evaluate their cost/schedule/quality impact; (3) Bring to CCB for approval or removal decision; (4) Update baselines for approved items; (5) Address the root cause — why did change control break down? Training, clearer processes, and management support for the PM's authority to enforce change control are needed to prevent recurrence.

**Why not A:** Terminating the project and restarting with a new team is an extreme and disproportionate response. The unauthorized additions can be addressed through retroactive change control and process reinforcement without discarding all work and team relationships.

**Why not B:** Simply accepting all unauthorized additions without evaluation undermines change control entirely. Each addition must be assessed for cost, schedule, and quality impact before a formal decision is made, as some additions may be detrimental to the project.

**Why not D:** Removing all unauthorized additions immediately could be wasteful and disruptive if some of the features provide genuine value. The proper approach is to evaluate each addition through the CCB process, which may approve some and reject others based on merit.
